It's rare that ultrasound would detect something not see on the mammo , however if you have dense breasts , that CAN front to a mammogram being smaller amount decisive. Ultrasound contained by these cases can identify lesions that are mammographically masked by dense tissue .
Without seeing your imagery it's hard to say aloud what it is that they are seeing via ultrasound .so that having be said , the only answer I can impart you is yes it COULD be malignant. More than likely it isn't , but here is the small percentage that it is .
In most cases like this you will be referred to a common surgeon , and they will either establish to do follow up imaging in 6 months , or more potential even recommend a ultrasound guided biopsy of the area. the biopsy will definitively explain to whether or not this is malignant or benign , and also whether or not this is just remaining blemish tissue from your previous finding 20 years ago. Since ultrasound moves through different tissue densities at different speeds ( it's all physics which i wont take in to ) and mark tissue is typically MORE dense than surrounding tissue it's possible that the scar tissue itself is what ultrasound is identify.
at this point it's up to you and your doctor to decide the subsequent course of action .

no med circumstance here, but maybe another solution. Digital mammograms are better at detection that the frail mammograms. But my wife has a thermal imaging of her breast instead of mammograms or ultrasounds. cancerous tissues show up within a different color. Maybe you can ask about this type of try-out, may give more answers. One file, insurance might not pay for this exam. We fought and won to have our company plan discharge for it. And the doctor was reluctant to charge the test instead of a mammogram. The skeptics enunciate the test have a 50% 'false positive' rate. but, if the 'false positive' spots are left surrounded by the breast after 10 yrs surprise! in plentiful cases it's cancer! so much for pushing early prevention.
It could extremely well me non cancerous But it could be lots other things .Until you get your experiment results you are just going to be speculating on what it is.It could be a caffeine pulp surrounded by there.Good Luck
